• ABOS Board Certified or TRULY Board Eligible in Orthopedic Sports Medicine (must become certified within 5 years) REQUIRED no exceptions
• Valid IL & IA license or Active IMLC Willing to Obtain Licenses - REQUIRED
• Strong arthroscopic and open sports surgery skills REQUIRED , shoulder, knee, hip PREFERRED
• Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice - REQUIRED
• DEA - REQUIRED
• BLS Certification and maintain recertification - REQUIRED
• Clean malpractice and license history HIGHLY PREFERRED / PREFERRED
• Does the provider require visa support No J-1 or H-1B (Note in Additional Submission Details) REQUIRED
• Submit Self-Query NPDB pulled within 30 days from submission at time of presentation - REQUIRED
• BLS
• Board Certified
• Board Eligible
