Job Summary

We are seeking an individual who can plan and coordinate care based on needs, evaluate the effectiveness of care provided identify and evaluate the significance of social, environmental, and emotional factors relative to identified health concerns. Our Client Service Specialists are expected to perform a variety of duties in the care for our clients including, care consultations with potential clients, Client/staff introductions, and quality assurance visits with our existing clients. This individual will evaluate the care plan through effective communication and in-person visits to ensure high-quality care, client satisfaction, and increased client engagement.Job Duties

  • Improve our clients' level of engagement, quality of life, happiness, and comfort
  • Meet with potential clients and family members to discuss needs and provide solutions develop service plans
  • Conduct service inquiries and care consultations as needed (consultative sales process)
  • Maintain ongoing communications and a consistent follow-up scheduled with potential clients
  • Maintain effective communication with team members to coordinate client's care
  • Conduct introductions with staff and clients
  • Evaluate and document the effectiveness of the client's care plan
  • Serve as the liaison between the client and the community, CAREGivers, and staff
  • Participate in the on-call rotation
  • Other Client Care duties as assignedThe ideal candidate will possess:
    • A passion for working with people and performing meaningful work
    • Excellent communication skills
    • Sound judgment and decision-making skills
    • The ability to work independently to meet challenging goals and deadlines
    • Advanced computer skills, including heavy use of Microsoft Office, Social Media, and Online Job Boards.
    • Creative strategies to attract and engage clients/client familiesJob Requirements
      • Associate's Degree or higher in a related field required
      • CMA, LPN a plus
      • 2+ year of in-home care, healthcare, or senior-related industry required (an equivalent combination of education & work experience may be considered for the right candidate)
      • Experience working collaboratively within a team of your peers to achieve client care success for the company
      • Must be willing to travel in the community (Quad City area) and surrounding areas. We service both Rock Island County and Scott County.
      • Must possess a valid driver's license, reliable transportation, and valid car insurance
