Robert Young Center - OutpatientLicensed CounselorFull-Time + BenefitsM-F, 8:00 AM - 5:00 PM - -The Licensed Mental Health Professional (LMHP) (Care Management) is supervised by the Manager, Outpatient Nursing Services Psychiatry and provides care to clients utilizing the Robert Young Center principles of Trauma Informed Care. The LMHP (Care Management) performs diagnostic assessment, coordinates treatment planning, and monitors clients through the treatment process. - The LMHP facilitates coordination of care with multidisciplinary groups to optimize individual treatment plans and safety of the behavioral health care client. - The LMHP provides supervision to Mental Health Professionals (MHP) and may train students assigned to the department. The LMHP provides crisis intervention, maintain accurate clinical documentation, and demonstrate Trinity's principles and values when interacting with clients, families, visitors, physicians and coworkers. - - -COVID-19 and Flu Vaccination Requirement: -It is required to be -vaccinated for COVID-19 and Influenza. Responsibilities:
Care Management:Assures that all clients receive clinically sound triage/referral and ongoing care management services for behavioral health needsPerforms diagnostic assessments and collects diagnostic information to determine an individualized treatment plan for the client.Plans treatment modalities, coordinates treatment plans and follows clients through the treatment processRequests consultation for clients that includes providers, social workers, and nursing staff when necessary.Evaluate the client's treatment plan through follow-up assessment and makes adjustments based on clinical findings.Identifies available support systems to meet the needs of each individual client.Delivers high quality care appropriate to the age and educational level of the client.Responds to crises situations by providing direction, recommendations, and crises intervention.Clinical Excellence:Maintains accurate, concise, current clinical documentation. - This includes but is not limited to all phone conversations, requests from the client, changes in the clinical conditions and all consultations with providers.Integrates ethical principles, evidence based practice, and research findings into all areas of practice.May provide psychiatric therapy to individuals, families, and/or groups.May provide Clinical and Reflective Supervision to RYC clinicians and nursing staff.Client Education:Utilizes a holistic approach to determine teaching methods appropriate to the individual client.Provides psycho-education to clients and families regarding their illness, medications, and/or treatment plan to supplement information provided by health care professionals.Evaluate the client's understanding of information provided and ability to follow through.

Qualifications:
Education:MSW, or MA, or MS from an accredited program with a minimum of two years supervised clinical experience. - Specialized training as appropriate to patient population, ie. Geriatric specialist, sexual abuse victims or perpetrators, marriage and family.License(s)/Certification(s):Independently licensed as a LCSW/LISW, LCPC/LMHC, or LMFT in the state of Illinois or Iowa according to the location of employment. - Basic Life Support (BLS) certification.Crisis Prevention Institute (CPI) training within 6 months of hire.Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS)Knowledge/Skills/Abilities:Knowledge of the addiction and specific attention to characteristics and behaviors of both individual and groupsKnowledge of computer software programs, as well as utilization of a case management program.Knowledge of modern drug/alcohol services and programsAbility to maintain a harmonious working relationship with various agencies, as well as departmental personnel.Ability to facilitate groups and individual treatment for adults.
